Completing the Digital Package

Special Steps to complete the package 

1. Complete each section on page 1 as this auto populates the rest of the paperwork.
   • All required fields will have a red asterisk in the field.  If not filled out you will not be able to submit the package.
2. On page 2 be sure to enter your student athlete’s “Grade in the Fall” and “School in the Fall“
   • AYF is requiring report cards to be turned in this year.  So please turn this into your team parent along with your AYF physical.
3. On page 6 there are 2 major section thats must be entered correctly.
   • In the Parent or Guardian Section please enter at least either the athlete’s Father, Mother or Guardian in this section.  This can not be left blank.
   • In the Emergency Medical Information Section please put an emergency contact other that someone already listed on the paperwork.
