I hereby grant permission for my athlete(s) to participate in the Hampton Roads Youth Sports Association (HRYSA) Sports Camp listed in the marketing material (Note: The name, date and time of the camp will be listed there.) I release the Hampton Roads Youth Sports Association (Hampton Roads Hokies) its Directors, Officers, Coaches, Managers and any volunteers jointly and severally of and from any and all claims, demands, rights and causes of action of whatsoever kind and nature, arising from or by any reason and for all loss or any death, sickness, injury or loss, theft or damage to personal property during his / her participation during the Camp whether such claim or liability arises from the conduct of any of the said persons organizing, supervising or conducting the activities of the Camp.

I further represent that my son or daughter will not participate in the Camp unless he or she is in good health and has no physical condition that would prevent him or her from participating in these activities.

I acknowledge that I have read this document and have had an opportunity to obtain a full explanation as to its contents. In consideration of the permission to participate in the Hampton Roads Youth Sports Association (Hampton Roads Hokies) Camp, I being the parent or legal guardian of the above athlete(s) hereby agree that the foregoing waiver and release shall be binding upon the child participant named herein.
