Each athlete participating in the 2023 National Championship games is required to complete a package.  Please see below some important details:

• Please enter the athlete's name in all areas referring to the participant.
• All Parent/Guardian sections are required fields for each participant.
• Signatures, Initials,, and Dates are required fields.
• If your child has any allergies or medical condition please note that in the field right below special circumstances.

• Team Level should show your one of the options listed below
  • All American 7U D2
  • All American 8U D2
  • All American 9U D2
• Enter you Athlete name and choose the head  coach from the drop down or write it in the paper you printed.
• List an emergency contact that is easily accessible in case of an emergency.
• For housing information, please list if you are staying in an AirBnB and the address to the home along with your arrival date.
